Application for Licensure Process

  1. Complete required licensure program
    Refer to the program of study
    Undergraduate programs require baccalaureate degree and minimum 2.75 G.P.A.
    Have Praxis II Test results sent to Malone
    Have BCI/FBI check completed
  2. Complete the Application for Licensure
    To obtain application forms click here.

    If applying for two licenses, two applications must be completed. If both applications are sent together, only an additional $20.00 is required.

    Submit with check to:

  3. Check status of certification
    Click Here. Upon entering your state ID number and last name, you can submit an inquiry on the status of your certification.
  4. Once licenses are issued you can check their status as the years go by Under Certification/Licensure select Individual Certification Data
    Fill in Social Security number or license number and last name
    Information regarding the status of your license will show including type, expiration, etc.

Allow four to six weeks for processing from the time above requirements were met and application materials submitted to Mrs. Gerber to the time the license arrives at your home.
