Injury & Sickness

• All traditional Undergraduate Students who are registered for at least 12 credit hours are automatically enrolled in the Student Accident and Sickness Insurance Plan. The insurance fee will automatically be placed on the student’s tuition bill.
• NEW FOR 2008/2009! You may opt to WAIVE OUT of the Malone plan by providing proof of comparable coverage. This can ONLY BE DONE ONLINE at www.studentinsurance.com.   No paper waivers will be accepted.
• Claim forms are available online or at the Student Health Center. For ICS claims (athletic injury), please see the Athletic Trainers.

• A waiver form may be completed to opt our of the college coverage by providing proof of comparable coverage.
• Insurance Waiver forms ane due in the Business Office by August 3rd of each year of attendance. Waiver forms are included in the fall pre-registration packets, and in the first fall bill as part of the Insurance Brochure.
• Claim forms are available on campus in the Student Health Center and from the Athletic Trainer.  You may also print a claim form from the Summit America Insurance Services website.
